Chart Self Audit

Please consider the following questions as you review a patient of your own with CKD.

1. What stage of CKD is it and does the problem listed in your problem list reflect the stage?
2. What is the cause of their CKD?
3. If diabetes, do they have other DM complications that corroborate the severity of the disease?
4. If they don't have diabetes what work-up has been done to consider other causes of CKD?
      (There is a great NEJM article about this differential diagnosis.).
5. Have you looked at urine microalbumin and urine micro in the last year?
6. Are they on an ACE-I?
7. How well controlled are their DM, HTN, lipids and smoking?
8. How well educated is the patient about their CKD?  Do they even know they have it?
9.  If they are CKD 3 or worse, have you screened them for anemia, hyperphosphatemia or Vit D deficiency within the last year?
10. Review their medication list and be sure that none should be renally dosed or are contraindicated at their GFR.
